Long Term Acute Care is often one step in a long road to recovery.  The case manager’s goal is to work with the patient, patient’s family, hospital and physicians to get the patient on the right course of management at the LTACH/LTCH, with the plan to move the patient to the next level of care or rehabilitation program after the LTACH/LTCH.  Case managers are a source of information about programs for the patient and family while in the LTACH/LTCH and following discharge. They assist the families in understanding the "big picture" regarding their loved one and help with communication among the disciplines and patients.

We are able to quickly meet the pharmaceutical needs of our patients through our on-site pharmacy including pain control, TPN and antibiotics.

Spiritual Services

Chaplain Richard Rist is available for spiritual services for our patients and families.  He leads a brief ecumenical worship service on Sunday afternoon beginning at 3:00 p.m. in the Dayroom.  Chaplain Rist welcomes all patients, family and staff who are available.  You can also expect to see Chaplain Rist at the following regularly scheduled hours:

Sunday 2:30 p.m. -6:30p.m.
Tuesday 8:00a.m.-2:15p.m.

Thursday 8:00a.m.-2:15p.m.

Social Worker

The social worker is on staff to help patients and their families since many of them need some type of guidance and counseling .  The social worker often works with the patient and family to develop a plan of action after the patient is discharged from St. Elias and can be instrumental in helping to find resources that can make life after St. Elias an easier transition.
