LTACH Admission Criteria

Any one or combination of the following are indicators that the patient is appropriate for LTACH Admission.

Patient is/has:

      Multi-system failure

      TPN

      CVA

      MRSA/VRE

Patient requires:

      Ventilator

      Extensive respiratory care/tracheostomy/chest tubes

      Cardiac monitoring

      Renal dialysis

      Extensive wound care

      IV therapy (antibiotic or cardiac)

      Daily physician visit

Other general indicators of a patient's LTACH eligibility:

      Patient is currently in ICU.

      Patient's medical needs are too extensive to transfer to lower level of care.

      Patient's rehab is compromised by acute medical issues.

      Patients at risk for medical complications upon discharge.

      Patient has history of multiple readmissions.
